Breast augmentation or breast enhancement is a surgical technique in which one’s breast size and shape is enhanced with either a saline breast implant or a silicone gel breast implant. According to the American Society for Aesthetic Plastic Surgery, breast augmentation is one of the most common procedures performed by women in the United States.
Multiple techniques or approaches to breast augmentation exist. One can insert a breast implant through a periareaolar (nipple), inframammary (within the breast fold), transaxillary (armpit), or a transumbilical (belly-button) approach. Dr. Grover performs breast augmentation using all of these techniques except for the transumbilical approach. Most procedures are performed with the periareareolar approach. When using the transaxillary approach, Dr. Grover prefers using the assistance of an endoscope to ensure adequate and symmetrical muscle release.
Breast implants can be in different positions relative to the chest muscles. They can be placed “above the muscle” (subglandular position), partially behind the pectoralis major muscle (partial retropectoral position), completely behind the pecoralis major and serratus muscles (total submuscular position), or in a dual-plane approach which is partially behind the muscle and partially on top of the muscle. Generally speaking, with the dual-plane approach, the top two-thirds of the breast implant is covered by muscle and the bottom third of the implant is not covered by muscle because the muscle has been released from its lower attachments to the chest wall. Dr. Grover utilizes the dual plane approach for most of his patients as he believes this technique allows for the most “natural appearing results” and optimizes the benefits of each pocket location. This technique allows for more natural appearing breast results, without giving the appearance of having implants that are placed “too high”.
Many choices exist when choosing breast implants. One may choose saline breast implants or silicone gel breast implants (if one qualifies for a cinical trial). Also, different textures of implants exist. One may use either smooth or textured implants. Breast implants also come in different shapes, round or anatomical (tear-drop shaped). Finally, many different “profiles” of breast implants exist. The profile of an implant relates the width of the implant to the amount of projection that the implant delivers. Breast implants come in “low”, “moderate”, “moderate-plus”, and “high” profiles. The most common type of implant used by plastic surgeons today is the smooth, round, moderate profile, saline implant. Dr. Grover selects the most appropriate type of implant for his patients on a case by case basis.
As with any device, breast implants may fail at some point in time although this is not very common. Saline breast implants may deflate and silicone gel implants may rupture. Most implants today are warranteed by the manufacturers for one’s lifetime.
Many of Dr. Grover’s breast augmentation patients experience a fairly rapid recovery with none or little bruising.

Mammaplasty – a general term for altering the size and/or shape of the breasts.
